To speed up your visit, you can print the forms below for your visit from this website and bring them to the office filled out. Please arrive 15 minutes early to fill out forms if not done already.

In addition to your driver’s license or photo identification, it is very important that you bring evidence of your insurance with you at each visit. Georgetown Ob/Gyn accepts most insurance plans.

If you are unable to bring evidence of your insurance coverage with you for our verification, we will ask you to pay for your first visit and give you a receipt to file for reimbursement with your health plan. We accept cash, pre-printed personal checks, debit cards, Visa, and MasterCard. All of our patients are required to pay their portion and/or co-pays at the time of their visit.
